Jay Kweskin and Connie Moore will highlight what a care manager does and how care managers can help your clients and families navigate through the many aspects of senior care. This presentation will show you the importance of seeing a broader picture of your client's needs, outside of the organizing needs you were called to assist with.

Our Presenters
Jay Kweskin is the owner and operator of KwesCare LLC. In 2014, Jay began KwesCare, a senior care management company that covers the St. Louis metropolitan area. KwesCare works with families and their older adult members as an advisor and advocate to assist with planning and management. Jay has his Bachelor of Science degree from Maryville University in St. Louis. He also has received his Certified Senior Advisor Certificate in 2014. Jay is the former national coordinator for the CSA Leaders Network and has served on the testing advisor committee and national conference programing committee for CSA. Jay is the chairman of the St. Louis Leaders Network and is also an associate member of the Aging Life Care Association. Jay is serving on the executive committee and board member for Covenant Place, a affordable community for older adults, a former board member of ARC Angels a organization for the prevention of suicide and a former development board member of Cardinal Glennon Children’s Hospital in St. Louis.
Connie Moore is the Principal and Founder of Concierge Care Management, which provides personalized service to individuals, their families and/or clients as they navigate complex situations and family dynamics when facing difficult decisions transitioning from one life stage to the next. Connie has served as the Certified Care Manager and Trust Officer for a major Midwest trust company for 26 years. She upholds her obligation of loyalty, trust and confidence in providing guidance on elder care/health care, and fiduciary decisions for you and/or members of your family. Connie holds a Masters in Counseling, degrees in Family Therapy and Gerontology, and the following certifications: Advanced Professional Certified Care Manager (CMC) and Certified Senior Advisor (CSA). Connie is a long-term member of Aging Life Care Assoication (ALCA) and Society of Certified Senior Advisors (SCSA).
